dc.description.abstract | Chronic diseases are the leading cause of illness, disability, and death in Australia (AUS) (1) These health conditions are complex and are influenced by lifestyle behaviours, poor medication adherence, lack of disease knowledge, and insufficient monitoring. (2) Therefore management requires a comprehensive patient-centred approach. Improvements to these factors can facilitate changes to an individual's health behaviours and thus reduce the risk of complications which lead to the long-term prognosis of the condition.
